PHOTOGRAPH RELEASE

Parent/Legal Guardian of the children listed on the registration form hereby give the Archdiocese of Philadelphia, its successors and assigns and those acting with its authority, the unqualified right and permission to reproduce, copyright, publish, circulate or otherwise use any school pictures and video of my child produced by the Archdiocese of Philadelphia and Pope John Paul II RCES & Community Camp. This authorization and release covers the use of said camp pictures and video in any published form, including but not limited to newsletters, websites, social media, and any other media of advertising publicity. I also understand that our school may be identified by name and I fully understand that this is a complete release of all claims against the Archdiocese of Philadelphia or any other person, firm or corporation by reason of any such use of such school pictures. I hereby warrant that I am free to give this permission. I also understand that Pope John Paul II RCES & Community Camp cannot be held responsible for photographs/video recordings taken by other parents during open performances.

CAMP PARTICIPATION AGREEMENT AND RELEASE
We (I), Parent/Legal Guardian of the child(ren) listed on the registration form
give permission for the child(ren) to participate in the Pope John Paul II Community Camp to which includes all related programs or events. In consideration for our (my) son’s/daughter’s participation and intending to be legally bound, we (I) and our (my) son/daughter agree and understand that we assume risk in full for our (my) son’s/daughter’s participation in the Pope John Paul II Community Camp and with full knowledge of the risk inherent in such an activity, we release and hold harmless Pope John Paul II Regional Catholic Elementary School, affiliated parishes, the Archdiocese of Philadelphia, and its priests, religious, teachers, aides, employees, agents, administrators, and any official representatives, from any and all liability, damages, or claims for any and all personal injury, bodily injury, and/or property damage arising or related to our (my) son’s/daughter’s participation. We (I) and our (my) son/daughter understand and agree to abide by all rules and regulations established by Pope John Paul II Regional Catholic Elementary School pertaining to the Pope John Paul II Community Camp and school code of conduct.

REFUND POLICY

Please note our new policy of NO REFUNDS will be given if your child misses a scheduled day of camp. Arrangements can be made with the camp manager regarding missed days. Please provide the camp with 24 hours notice if your child cannot make a scheduled day of camp. ANY camper dismissed for conduct, or who withdraws from camp voluntarily, for any reason, will not receive a refund. See Camp Code of Conduct.

CAMP CODE OF CONDUCT

1. Respect for self.
2. Respect for others.
3. Respect for the environment.

The list of behaviors which violate the camp code of conduct is by no means all inclusive. It is impossible to designate a given infraction and give it the same weight under all circumstances. The camp director is the final recourse in all disciplinary situations and may waive any and all regulations for just cause at her discretion. The common good of the entire camp community needs to be valued by all. Choices that infringe upon this end must be dealt with in a fair and just manner. Refusal to accept consequences for behavior and choices which violate camp policy can result in the dismissal of the family from Pope John Paul II Regional Catholic Elementary School Summer Camp.

1. Respect for self: Students show respect for themselves behaving in safe and appropriate ways while on school grounds and in locations where they represent the camp (bus, field trips).

2. Respect for others: Students show respect for others by showing good manners, listening to and following directions and instructions given by adults in the building. Keeping hands, etc. to themselves and allowing others to be safe at our camp and in all locations where they represent the camp. Using respectful verbal and written language when addressing all others. Respecting the rights of others, including the right to attend summer camp in an orderly and disciplined atmosphere free from verbal, physical or sexual harassment

3. Respect for the environment: Students show respect for our environment by keeping their materials neat and organized. Using community materials respectfully and returning them appropriately. Keeping school surfaces (walls, desks, etc.) clear of writing and graffiti. Being respectful of bathroom spaces in order to keep the space sanitary. Eating respectfully and cleaning up floors and tables while at lunch or snack time.

Behavior System will run according to the following:

Warning 1-Phone call home.

Warning 2-Phone call home, and child(ren) will need to be picked up from camp.

Warning 3-Dismissal from camp. No refunds or credits will be given.

Please note: Egregious behavior issues could be subject to immediate dismissal.
